For participation in sports, please provide to the Health Office a complete physical examination by a health care practitioner prior to the first game. The examination should include the provider's clearance to participate.
Currently, ONE physical exam is sufficient for the 6th-8th grades.
You will need to upload health forms to Final Forms. Our Athletics Department will email notifications as the Fall draws near.
