HEALTH REQUIREMENTS

The Middle School requires an updated physical (dated within one year), if your student is planning to participate in school sports. Currently, ONE physical exam is sufficient for the 6th-8th grades. If your child has changes to their medical status, please update the Health Office. You may upload physicals through PowerSchool. You may also fax them to the school.

The Middle School is required by law to have a copy of required immunizations for school entry.

All 7th Graders are required to have a Tdap vaccine on file.

New Hampshire law He-P 301.13-15 requires that all children enrolled in any school, pre-school, or child care have certain immunizations to protect them and those around them from vaccine preventable diseases. In addition, schools and child care providers must collect and review the immunization records of enrolled children and submit an annual immunization report to the DHHS (RSA 141-C:20-e).
