Please inform the Health Office of any medical conditions, health concerns, or allergies (especially life-threatening) that may impact your child at school.

• If your child takes a daily medication to be administered at school, please inform the Health Office.

• If your child has asthma requiring an inhaler at school, please provide an asthma action plan from your Pediatrician.

• Some medical diagnoses may also require an Emergency Action Plan signed by a physician (ex: Seizure disorder, Life-threatening allergies, Diabetes, etc). PLEASE NOTIFY THE HEALTH OFFICE.

• All student medications must be in the original labeled container.
