New Client Paperwork

Please fill out, save and return the following forms to me via email along with a copy of your insurance card.

You will need to click on the four document buttons below. Each button should open a new window. Once the document has loaded in a new window, click on the printer icon on the top right to activate the fillable fields. Once you have filled it out, you must save it to your computer and then email it to me.

The first two documents can be done paperless (no printer necessary). The third document requires a signature. You can print it out and sign it and scan it and send it back to me or I can send you a request through an e-signature service. The fourth button is a link to the HIPAA notice. The HIPAA just needs to be reviewed; there is nothing that needs to be filled out or signed.
