7th-Grade Registration Date

• Wednesday, May 4 at 4:00 pm - 8:00 pm

Registration is now closed for all incoming grade levels. Kaimuki Middle Summer School will be sending out course confirmations by May 20, Friday, to the designated email that you used to fill out your registration form.

PLEASE NOTE: Due to reduced staff and adhering to COVID-19 safety measures, Kaimuki Middle School will only be accepting a limited amount of students, and registration is based on a first-come, first-served basis. Registration dates and times will be strictly adhered to.

REQUIRED INFORMATION FOR REGISTRATION FORM

Please have your registration information ready prior to your designated registration date and time. Any changes that must be made after you have completed a registration form WILL REQUIRE you to re-register and your child might lose their place for enrollment.

  • 10-Digit Student Identification Number

  • Child's home address and phone number

  • Child's date of birth

  • Did your child receive special education services for the school year 2020-2021?

• Parent/Guardian Information

  • Name, phone number, and employer name

  • Emergency contact, phone number and relationship

  • Email address for school-related confirmations, announcements, and emails

• Course Selection (you will be asked to select four classes, in order of preference)
