Physical Examinations and Immunizations

Required Immunizations : All children entering 7th grade (including currently enrolled students)

Exceptions will be granted to any child whose parent or guardian shall file with school authorities a signed, written statement that such measures conflict with his/her religious tenets and practices; or due to medical reasons if such child has a written statement from his/her doctor excusing him from such immunization.

Proof of exceptions will be in writing and filed in the same manner as other immunization records.

• Tetanus-diphtheria-pertussis booster (“Tdap”)– not required if a Td booster dose given less than 5 years before 7th grade entry is recorded on the DTaP/Td line

• Verification of immunity to varicella – 2 doses or history of disease
