It is important that we have current contact information for your child. To update your address, phone number, email address, and/or emergency contact(s), please complete the Student Information Update Form.
Please provide a copy of your ID when updating information.
Address changes require the following documentation:
REQUIRED: Rental or lease agreement, mortgage document, deed, escrow document, or real property assessment document in the parent/guardian’s or friend/relative’s name
REQUIRED: Utility bill in the parent/guardian’s or friend/relative’s name (e.g., water, electricity, gas, telephone, cable, internet)
If parents are living with a friend/relative, the following ADDITIONAL document is required:
Notarized statement signed by the friend/relative that describes the living arrangements. The statement is valid for a maximum of one calendar year.
Name and guardianship changes require proper legal documentation.
