Commitment of at least 100 hours
Two-step TB (tuberculosis) clearance
Orientation session attendance
Parental permission, MMR and Varicella Immunization
Record (for 15 to 17-year-olds)
Agreement to serve all of our patients with C.A.R.E. (Compassion, Aloha, Respect and Excellence)
