Transition Services is a coordinated set of activities for a student, based on individual student’s needs, preferences, and interests, that promotes movement from school to post-school activities program for students in Special Education ages 16-22. It is designed to promote career awareness and exploration while students complete their secondary education program.
Students are provided opportunities for volunteering, paid work experience, job coaching, and ongoing support and guidance from vocational personnel. Vocational assessment, counseling, guidance, and career assessment services are provided through the Individual Education Program (IEP). Work Based Learning, Workplace Readiness and Self Advocacy Training are provided through a partnership with the Department of Rehabilitation and the district has a large Workability grant that supports job readiness, career exploration, support with college matriculation, work based learning and transition planning for every student with an IEP.
As students graduate with a diploma or certificate of completion from JUHSD, Transition Services supports with post secondary planning, program tours at the local community colleges, trade schools, vocational programs, and GGRC adult programs. Transition Specialists work closely with students and families to create a smooth transition from our district to continuing education, competitive integrated employment and independence as an adult.
Our Role
Provide direct student services from 9th grade through exit IEP & Eligibility Awareness, Job Readiness, Self Awareness & Self Determination, Self Advocacy Training
Provide data to case managers by conducting student transition questionnaire, career interest inventory, student interviews
Support students with job placement and provide job coaching
Activities to support self-knowledge and self-advocacy
Maintain grants
Provide community based activities such as job exploration and travel training
Developing job sites based on student interest
College matriculation support
Support coordination between families and teams with agency linkages (GGRC, DOR)
