- Kindergarten Families
if you haven't done so already, fill out the Transportation Verification Form here ASAP.
Don't forget to send in your required medical documents! Hard copies can be mailed or dropped off at 2301 Clarendon Avenue, or you can email a legible copy to hawkoffice@jefferson.kyschools.us.
Documents to include:
State Certified Birth Certificate (a copy is fine)
Initial Vision Exam - must be from an optometrist/ophthalmologist (screening from a pediatrician is not valid)
School Messenger is the phone, email, and SMS messaging system JCPS uses to communicate school cancellations, safety alerts, and other important information to you.
Our school utilizes the School Messenger system to deliver important information via text message straight to your mobile phone. We encourage all incoming kindergarten and new-to-JCPS families to opt in to receive text alerts. Please see the link above for how to opt in, or text "Yes" or "Y" to 67587.

Classroom teachers will send sign-up information for Class Dojo
The JCPS Parent Portal (run by Infinite Campus) allows you to track your child’s academic progress and develop a partnership with your child’s teachers. Infinite Campus is a secure and convenient way to monitor your child’s attendance, assignments, daily grades, and class schedule, and health information.
Please review the dress code policy provided with the link above. Students must not wear open-toed shoes!
- Before & After-school Childcare
Before & After-school childcare is offered at Hawthorne through the YMCA child enrichment program (CEP), more information about the program can be found on their website here: YMCA CEP.

Every year parents are required to fill out several forms for each attending student. In the past, these forms were given to parents during back-to-school events or in a paper packet on the first day of school. However, in 2020 the district moved to a digital format.
All REQUIRED school forms can be found on the Parent Place website (not to be confused with Parent Portal).
Please allow 15 minutes to complete per child.

Access Parent Place from a desktop or laptop computer only -- smartphones typically are not compatible
Use a Chrome browser ONLY
Within the Chrome browser, click on the 3 dots at the top-right corner of your screen and click "New Incognito Window."

Medications at School
Please DO NOT send any medications to school with your child. This includes all over-the-counter medications and cough drops. ALL medications require a physician’s approval. Medication forms can be printed here: JCPS Medication and School Health Plan forms. Please contact us if your child requires daily or emergency/as-needed medications during the school day.
A few reminders...
Custody Papers
If there are any circumstances relating to custody, visitation, or protective orders, please provide the front office with current court documents each school year and when changes occur during the school year.
Student Absences
Please call the school office each day your student is absent. Please send a written explanation of the absence on the day your student returns to school; these may also be emailed to our attendance clerk, Ms. Lisa Fields. Absence notes must be received within 3 days of the absence to be considered for excusal. If you know your child will be absent due to an upcoming event or travel, please inform the school office in advance. Some absences may be eligible for approved educational leave. These must be submitted on the appropriate form and approved by the principal at least 2 weeks before the absence.
Transportation Changes
All children will be dismissed according to the transportation plan on file. If dismissal plans must change for a particular day, please submit those changes in writing the day of or in advance of this change. There may be some instances where a change arises during the school day. If this occurs, please contact the school office(hawkoffice@jefferson.kyschools.us) before 1:30 pm so the student and teacher can be adequately notified of the change.
The mission of the Bloom/Hawthorne Family Resource Center (BHFRC) is to promote the flow of services, resources and supports to families in ways that strengthen their functioning and help build a bridge between the school, home, and community. We work with students and their families to help enhance the growth and development of individual members and the family unit as they learn to shape the social and environmental forces affecting their daily lives.
We believe that by helping to remove barriers that interfere with learning, we can help our students achieve and succeed, becoming productive members of society.
