The School District of Janesville offers the following voluntary benefits for employees who work 20 or more hours per week
Lincoln Short Term Disability
Provides a cash benefit when you are out of work for up to 11 weeks due to injury, illness, surgery, or recovery from childbirth and provides a partial cash benefit if you can only do part of your job or work part time.
Weekly benefit amount
60% of your weekly salary, limited to $1,000 per week
Sickness/Accident Elimination Period
You must be out of work for 14 days due to an illness before you can collect disability benefits. You can begin collecting benefits on day 15.

Lincoln Term Life Insurance Plan
Provides a cash benefit to your loved ones in the event of your death.
Employee:
Initial Open Enrollment: When you are first offered this coverage, you can choose a coverage amount up to $150,000 without providing evidence of insurability.
Annual Limited Enrollment: If you are a continuing employee, you can increase your coverage amount by $10,000 or $20,000 without providing evidence of insurability . If you submitted evidence of insurability in the past and were declined for medical reasons, you may be required to submit evidence of insurability.
Maximum Life Insurance Coverage Amount:
• You can choose a coverage amount up to 5 times your annual salary ($500,000 maximum) with evidence of insurability.
• The maximum coverage amount for employees 70 and older who are electing coverage for the first time is$50,000.
• Your coverage amount will reduce by 33% when you reach age 65 and an additional 17% of the original amount when you reach age 70.
Spouse:
Initial Open Enrollment: When you are first offered this coverage, you can choose a coverage amount up to 50% of your coverage amount ($50,000 maximum) for your spouse without providing evidence of insurability.
Annual Limited Enrollment: If you are a continuing employee, you can increase the coverage amount for your spouse by $5,000 or $10,000 without providing evidence of insurability. If you submitted evidence of insurability in the past and were declined for medical reasons, you may be required to submit evidence of insurability.
Maximum Life Insurance Coverage Amount:
• You can choose a coverage amount up to 50% of your coverage amount ($250,000 maximum) for your spouse with evidence of insurability.
If you are diagnosed with a critical illness, critical illness insurance can help you pay for expenses that aren’t covered by your existing health insurance plan. Critical illness coverage pays you a lump-sum cash benefit to help pay for treatment or bills, and you can add a wellness benefit option to help cover the cost of health screening tests.
Some covered illnesses include heart attack, stroke, bypass surgery, major organ transplant, and end stage renal failure.
Benefit Amount:
Employee: $15,000 or $30,000 guaranteed issue coverage
Spouse: 100% of employee benefit amount
Child(ren): 25% of employee benefit amount

If you are accidentally injured, accident insurance can help you take care of out-of-pocket expenses and medical costs beyond what your existing health insurance plan covers.
Some covered accident insurance benefits include: hospital confinement, ambulance bills, dislocation or fractures, accidental death and dismemberment, and medical expenses.
