T dap (Tetanus, diphtheria, acellular pertussis): 1 dose required and given at 11 years old
Please submit a copy of your child's NJIIS form or physician's/APN copy directly to school nurse.
All children attending Pre-K are required according to the NJ Department of Health to get an annual flu vaccine from September, 2020 thru December 31, 2020. The CDC recommends flu vaccination for all starting at age 6 months old.
Please send proof of vaccination directly to the school nurse.
Every school year your child needs a new asthma treatment plan, allergy action plan, seizure action plan, or medication orders.
Please contact the school nurse to set up an appointment to drop off medication with appropriate medical forms (see School Forms Page).
Fever. Any temperature of 100.4° is considered a fever. Your child must be fever-free for 24 hours WITHOUT fever reducing medication before they can return. Even with a temperature of 99.0 or higher, be mindful of how your child is feeling and acting, and consider keeping your child home.
Vomiting or diarrhea. Your child must be free of any episodes for 24 hours before they can return.
Coughing that won’t stop. A bad cough not only makes it difficult for your child to concentrate or do school work, but it usually warrants a call to the doctor. Any breathing difficulty or out of the normal for you child should be checked out by your physician.
Contagious diseases. Certain illnesses and rashes like flu, strep throat, pink eye, hand, foot and mouth disease, chickenpox, and measles, are highly contagious. Keep your child home until advised by a doctor they can return. Length of absence varies depending on the illness. Please alert the school nurse if your child has a communicable disease.
Please reinforce proper hand washing and cough or sneezing hygiene with your child to help reduce the spread of germs.

Height and Weight for all students in Grades PK through Grade 5
Blood Pressure for all students in Grades K, 1, 2, 3, 4, 5 annually
Dental Screening for all students in Pre K annually
Vision Screenings for all students in grades Pre-K, K, 1, 2, 4 annually.
Hearing screenings will be performed in grades Pre-K, K, 1, 2, 3 annually
Parent Excuses: A parent or guardian may excuse their child, via a written note, for up to 2 days. Anything beyond 2 days will require a doctor note.
Doctor Excuses: Submit all doctor notes directly to the nurse. The doctor note should include dates of exclusion. Any note that states "excused until further notice" will require a new doctor note with clearance in order for the student to return to participating in gym.
Please note: Any student excused from gym will not be allowed to participate in recess. They will remain indoors in the cafeteria.
