Medical treatment plans and medication administration forms expire every year!
Please have your child's doctor complete new medical treatment plans and medication administration forms. Then submit all forms, along with the medications, to the school nurse ASAP. (Click here to review medication guidelines and retrieve copies of our school forms!)
ALL INCOMING 6TH GRADERS must submit proof of an up to date vaccination record in order to start school in September! Additionally, Meningococcal and Tdap vaccines are required for those entering 6th graders.
From the State of New Jersey Department of Health: Meningococcal and Tdap vaccines are required for all entering 6th graders who are 11 years of age or older. If in 6th grade and under age 11, children must receive the vaccines within 2 weeks of their 11th birthday. (Click here for more information)
GMB offers three seasons of sports to all of our students. All students will require a current sports physical and forms in order to participate.

Parent Excuses. A parent/guardian may excuse their child, via a written note, for up to 2 days. Anything beyond 2 days will require a doctor note.
Doctor Excuses. Submit all doctor notes directly to the nurse. The doctor note should include dates of exclusion.
NOTICE:
Any note that states "excused until further notice" will require a new doctor note with clearance in order for the student to return to participating in gym.
Any student excused from gym will not be allowed to participate in recess or sports. They will remain indoors on the bleachers, unless otherwise specified by the child's doctor.
Fever. Any temperature of 100.4° is considered a fever. Your child must be fever-free for 24 hours WITHOUT fever reducing medication before they can return. Even with a temperature of 99.0 or higher, be mindful of how your child is feeling and acting, and consider keeping your child home.
Vomiting or diarrhea. Your child must be free of any episodes for 24 hours before they can return.
Coughing that won’t stop. A bad cough not only makes it difficult for your child to concentrate/work, but it usually warrants a call to the doctor. Any breathing difficulty or out of the norm for you child should be checked out by your physician.
Contagious diseases. Certain illnesses and rashes (for example: flu, strep throat, pink eye, hand, foot and mouth disease, chickenpox, and measles) are highly contagious. Keep your child home until advised by a doctor they can return. Length of absence varies depending on the illness. Please alert the school nurse if your child has a communicable disease.
Please reinforce proper hand washing and cough/sneezing hygiene with your child to help reduce the spread of germs!
Each year students will receive height, weight, and blood pressure screenings.
Vision screenings will be performed in 6th and 8th grade.
Hearing screenings will be performed in 7th grade.
Scoliosis screenings will be performed in 7th grade. Please indicate on your emergency health form whether you would like your student to receive a scoliosis screening or not.
Physical exams are provided in 7th grade by our school physician to students who do not have access to a primary medical provider. Please indicate on your emergency health form whether you would like your student to receive a school based physical.
Parents/guardians will be contacted and referrals will be sent home if it is recommended to follow up with your physician for further screening.
