The potential for the spread of infectious diseases is increased whenever individuals are brought into group settings. To prevent some of the most serious infections, the New Jersey school immunization rules, Immunization of Pupils in Schools (N.J.A.C. 8:57-4) require students to receive a series of immunizations prior to attendance at school. In addition, schools are required to enforce requirements, maintain records, and submit annual reports to the state and their respective local health department. The following materials provide guidance and tools for carrying out these requirements.
Provides immunizations for children age 5 years through 18 years who have no insurance or whose insurance does not provide for the cost of immunizations. You must bring the child’s immunization records so that the medical staff will know what immunizations the child needs. A parent or legal guardian must come with the child. There is a fee for this clinic.

Diphtheria, tetanus, and pertussis (D.T.P.) - 4 doses Inactivated Polio (Polio) - 3 doses
Haemophilis influenza type b (Hib)- at least one dose given on or after the first birthday.
Pneumococcal conjugate (PCV-13)- at least one dose given on or after the first birthday.
Measles, Mumps, Rubella (MMR)- one dose given at 15 months or greater.
Varicella (VAR) - one dose given at least one dose at 18 months or older
Influenza - the current seasonal flu vaccine is required every year by December 31 for children 6 - 59 months of age

6th grade: 1 dose of Tdap and 1 dose of Meningococcal are due at 11 years old and required to enter 6th grade
