Registration Forms

Prior to February 12, 2022

Please send a typed list of participants and alternates (listed as alternates) to Dan by the end of January. This will facilitate having correctly spelled names on certificates!

On the morning of February 12, 2022

There are three registration forms that must be filled out in their entirety and submitted during registration on the morning of February 12, 2022. Registration will be open from 7 AM - 8:30 AM. Please click on the name of the form to open and print the document. Please note, on the morning of Feb 12, we would be happy to tell you where your homeroom will be, however, you will not be allowed to register your team until all paperwork is complete. If you would prefer, paperwork can be submitted in advance of the competition through Thursday February 10, 2022.

- Basic Registration Form: One form per team; must be signed by the school's principal. Any team arriving without a properly filled out and signed form will not be allowed to compete.

- Code of Conduct Form: One form per team; must be signed by all coaches, students (competing and alternate) and observers. Any team arriving without a properly filled out and signed form will not be allowed to compete. Remember - Our regional has grown, so we will have more teams to help throughout the day than ever before. We ask that you have all potential parent and other observers sign the form prior to Saturday morning. It is okay to have extra people sign it, then only some show up. With so many more teams, it will not be possible for us to have folks dropping by to sign it throughout the day, and without signing it, folks cannot stay and observe. Thanks for your help with this! A Change since 2019: Ivy Tech has expanded our "working with minors" policy - all adults working with minors on our campus must be checked against the national sex offenders database (https://www.nsopw.gov/en/). As we will not have the names of your observers / parent volunteers until the morning of the competition, we request that you take a few minutes to check them prior to their attendance. If you need more information on how to perform this check, please let us know.

- Health Information and Liability Waiver: One form per student; must be signed by each student's parents. This form combines the emergency medical information, liability waiver and photo release. Any student arriving without a properly signed liability form will not be allowed to complete.
