Medications & Physician Authorization

Drop Off Time: 7:30-8:00am

On the first day of camp, please park and walk your camper inside to check-in with medication and paperwork with the Nurse.

Bring to Health Room:

1. A Physician's Report--Medication Form filled out and signed by you and your camper's physician.

2. Medication (PRESCRIBED OR OVER THE COUNTER) must be brought to school by a parent or other responsible adult and must be labeled as follows:

   • Name of student

   • Dosage of medication

   • Name of medication

   • Time medication is to be given

To provide a safe environment for your child and other students, all medication must be stored by the Career Academy nurse. Your child will report to the health room when he/she is scheduled to take the medication.

Any unused medication will be destroyed unless a parent comes to school to pick it up no later than 3:45pm on the last day of camp.
