School Immunizations

Minnesota School Immunization Law

Incoming kindergarten students need (4) DTap, (3) Hepatitis B, (4) Polio, (2) MMR, and (2) Varicella vaccinations completed and on file in the health office.

Seventh grade students need to have a Tdap and a Meningococcal vaccine completed prior to school beginning and on file in the health office. They must also have record of (3) Hepatitis B, (3) Polio, (2) MMR, and (2) Varicella vaccinations on file in the health office.

Students entering 12th grade need to have documentation or a legal exemption for the booster dose of meningococcal vaccine (Most children received the initial dose of this vaccine when they entered 7th grade). Teenagers and young adults are at higher risk for meningococcal disease. This dose is important to make sure they are fully protected through their highest-risk years.

We are providing our parents and guardians with the information and time to get their child’s shot records current. Fall will be here before we know it and ultimately it is the responsibility of parents and guardians to ensure their students will not miss any classes because a shot record is not up to date. 

Please submit a copy of these records or an exemption to the school as soon as possible, if this has not already been done. Students cannot attend school if the records are not on file in the Nurse's Office.

Immunization Exemption Form: A medical exemption is completed if your child has a medical contraindication or laboratory evidence of immunity (e.g. had chicken pox). A physician signature is required.

A conscientious exemption is completed if receiving an immunization is contrary to the conscientiously held beliefs of his/her parent or guardian. A parent or legal guardian must complete and sign this form and have it notarized. This form can be notarized by Mrs. (Marcia) Schultz in the District Office.

Varicella Vaccine

**Effective on September 1, 2010, for children enrolling in kindergarten and seventh grade, Minnesota schools can no longer accept a parent/guardian's signature as proof that a child has had the chickenpox. We are now required to get a doctor's signature stating the child has already had the disease, have a medical reason for not getting the vaccine, or that the child is already immune to chickenpox. Space is provided on the back of this form for the physician's signature.
