You've identified a member that would like ElliQ... now what?
Upcoming Waitlist Process:
We have seen so much success with the ElliQ program that we will soon have to implement a waitlist for interested users.
How will this change our usual process for getting your clients an ElliQ?
Not much will change at all! This is the process moving forward:
Submit designation forms for your interested client as normal
We will contact you to let you know whether a unit is available or not
If we have units, we will fill the order immediately.
If there is a wait, we will let you know, and when a unit becomes available we will reach out to you in order to confirm whether your client is still interested. The wait may be up to several weeks.
Your client will be shipped their ElliQ

Once your client has decided to welcome ElliQ into their home, the case manager must complete the Designation Form on their behalf. This form allows us to allocate a specific device ID to the individual so that we can personalize the device to the client and to have you set up as a contact.
Please allow 24 hours between submitting the designation form and bringing the ElliQ to your client.
