Driver Checklist

Required Items

Hello! Thank you for saying yes to the call to volunteer as a driver! In order to maintain the safety of our parish and our volunteers, we do have a few onboarding tasks that must be completed by all volunteer drivers. All volunteer drivers are required to complete defensive driver training, sign the volunteer driver agreement, pass a driving history check, and submit copies of their drivers license and insurance card prior to driving for the parish. Please complete form to sign-up. A link to the online module will be sent to you.

Please complete and submit to your supervisor or christineh@ihmco.org.

Driver's License and Insurance Card

Please submit legible copies of front and back (or bring originals to make copies) of your license and insurance card to your supervisor or the parish office.

Criminal Background Check Set-Up

Driving History Check

Please complete form to set-up if you haven't done so already. A secure link will be sent to you to submit the rest of your info for a background check.

Defensive Driver Training

Per archdiocesan regulations, all volunteer drivers must complete an online defensive driving module prior to driving. Please provide the below information so that we can get an account set up for you by the archdiocese. You will receive a link from a third-party called Zywave to access your account and complete the course. The course should only take about 45 minutes to complete.

To get credit for the course, you must do the following:

Complete the module
Print a copy of your certificate and turn it in to Immaculate Heart of Mary

Volunteer Driver Agreement

Please complete and sign the volunteer driver agreement and submit it to your supervisor or katelyns@ihmco.org.

Drivers License & Insurance Card

Please submit clear, legible copies of the front and back of your valid drivers license and insurance card to your supervisor. You can also bring in the originals to the parish and we will make copies for you

Driving History Check

All drivers must pass a driver history check. If you have not already completed the attached set-up form for a criminal background check, please do so, and make sure you check "yes" to needing a driver history check.

If you do not already have a criminal background check on file, a link will be sent to you by Fastrax to complete your info for both checks. If we do already have a criminal background check on file, we will run the driver history check based on this form.
