Scoliosis Screening

Scoliosis screening at school takes place in 5th & 7th grades. For more  information on why this is needed and what the screening process involves, please refer to the resource below obtained from Children's Healthcare of Atlanta.  Before the screening is conducted, a parent letter will go out to all of the students in grades 5 & 7.  As a parent or guardian you may choose to opt out of the school screening if your child has been previously screened during their annual physical with their primary care doctor.  If your child has previously been screened and you would not like your child to be screened at school, please fill out the following form and return it to the clinic as soon as possible.  (*Please see page 2 of the form for more information from the Georgia Department of Public Health.)
