Please notify the school with any health concerns that may require the use of medication(s) while your child is at school. Depending on the condition, a meeting may be scheduled to go discuss the condition and plan of care.
ICSAtlanta does NOT supply any medications (prescription or over-the-counter) to students. All medication must be brought to the school by a parent or guardian for proper storage in the clinic.
The clinic is not permitted to administer medication(s) without having the completed corresponding authorization form.
Students are NOT allowed to to carry medications in their backpack or have medication in their possession, unless having a physician’s order to self-carry emergency/rescue medications (ex: life-saving, emergency medications like epinephrine, glucagon, or rescue inhalers). The self-carry authorization form must be completed and signed by the prescribing doctor.
Over-the-counter (OTC) medication must be brought the clinic in its original container, labeled with the student’s name, and with the completed OTC authorization form. OTC forms require parent signature.
Prescription (Rx) medications must be in the their original containers, labeled with the child’s name, name of the medication, expiration date, prescribing physician's name, name of the pharmacy filling the prescription, and times(s) the medication is to be given while at school. Prescription authorization forms require signatures from both a parent and the prescribing physician. *New Rx authorization forms are required to be completed for EACH school year.
Any medication(s) which can be given at home(ex. first thing in the morning, right after school), please do so to avoid increased exposure risk to sick children while in the school clinic.
It will be the responsibility of the parent/guardian to inform the school of any changes in your child's health or medication requirements.
