New students, and any returning students with new health concerns/conditions (ex: diabetes, asthma, food allergies, seizures, etc.), are required to complete the appropriate medical paperwork.  This paperwork is to be provided to the clinic at the beginning of every school year.  *Email or bring in completed paperwork, along with any corresponding medication(s), to the nurse either at the open house event or on the first day of school. 

The appropriate campus nurse will reach out via email to parents of new students or returning students with new health concerns/conditions to schedule a meeting-phone call or virtual video call-before the start of the school year to discuss the condition(s) and all pertinent health  information.
