IB required forms

The following forms can be found in the Forms/Ecollect part of the student and parent portal in PowerSchool.

• IBDP Coursework Consent and Declaration form

  • This is a general form where students and their parents agree to let teachers and the IBDPC upload their work onto the IB's system. It also has a blanket statement affirming that all the work the student is submitting is their own work and has not engaged in any malpractice in the creation of their work.

• Notification of copyright and extension of license

  • This form notifies students that they own the copyright to work that they submit to the IB. It also requires students to give the IB and ICS permission to use their work for the purposes of training and teaching.

• Student score release form for universities

  • This is a form meant to be filled out in April/May of grade 12, when students generally know which university or possible universities that they will attend the following school year. To fill out the form students need to have contact information for the university's admissions office as well as a their university application ID and/or student number.
