Implementation Guidance

Managing Healthcare Needs Policy
Yn niweddar mae Llywodraeth Cymru wedi ailddatgan yr hyn maent yn ddisgwyl gan ysgolion, awdurdodau lleol a byrddau iechyd lleol mewn perthynas â'r cymorth a ddarperir i blant a phobl ifanc ag anghenion gofal iechyd ym mhob lleoliad addysg a gynhelir yng Nghymru, gan gynnwys bod angen pennu safbwynt polisi clir ar y mater hwn. Mae hyn yn hanfodol i sicrhau bod plant a phobl ifanc ag anghenion gofal iechyd yn gallu cael addysg sy'n ddiogel, yn deg a heb darfu diangen.
Bwriad y disgwyliadau polisi a nodir yn y llythyr hwn yw ailddatgan ein hymrwymiad i'r canlynol:
Sicrhau nad yw dysgwyr ag anghenion gofal iechyd dan anfantais o ran cael mynediad at addysg neu gymryd rhan mewn gweithgareddau ysgol.
Sefydlu trefniadau cyson o ansawdd uchel ar gyfer nodi, cynllunio a darparu cymorth gofal iechyd mewn lleoliadau addysg a gynhelir.
Cryfhau'r cydweithredu rhwng ysgolion, awdurdodau lleol, byrddau iechyd, a theuluoedd.
Hyrwyddo arferion diogel a phriodol sy'n seiliedig ar dystiolaeth wrth reoli anghenion gofal iechyd mewn lleoliadau addysg a gynhelir.
Sicrhau bod hawliau, urddas a lles dysgwyr yn cael eu blaenoriaethu bob amser.
Mae'r canllawiau Cefnogi dysgwyr ag anghenion gofal iechyd (2017) a Rhan 2 o'r Fframwaith ar gyfer Gwasanaeth Nyrsio mewn Ysgolion (2018) yn nodi'r cyfrifoldebau statudol ac anstatudol ar ysgolion, awdurdodau lleol a byrddau iechyd lleol mewn perthynas â sut y dylid cefnogi dysgwyr ag anghenion gofal iechyd. Gyda'i gilydd, mae'r dogfennau canllaw hyn yn disgrifio sut y dylai partneriaid weithio gyda'i gilydd i sicrhau bod anghenion dysgwyr yn cael eu deall yn glir a bod cymorth priodol a chydgysylltiedig yn cael ei roi ar waith. Rhoddir crynodeb o'r prif gyfrifoldebau isod.
Mae Llywodraeth Cymru yn disgwyl i'r holl bartneriaid ddeall a chyflawni'r cyfrifoldebau hyn, gan sicrhau bod anghenion a buddiannau gorau'r plentyn yn parhau i fod wrth wraidd y broses bob amser.
Yr hyn a ddisgwylir gan ysgolion ac awdurdodau lleol
Mae'r disgwyliadau hyn yn berthnasol mewn ysgolion arbennig ac i blant sydd angen cymorth gofal iechyd mewn ysgolion prif ffrwd. Gellir crynhoi'r disgwyliadau hyn fel a ganlyn:
Polisi gofal iechyd: Rhaid i bob ysgol ddatblygu, cynnal a chyhoeddi Polisi Gofal Iechyd sy'n amlinellu trefniadau ar gyfer cefnogi dysgwyr ag anghenion gofal iechyd. Gweler templed polisi ynghlwm.
Llywodraethu a hyfforddiant: Rhaid i drefniadau llywodraethu fod ar waith i sicrhau bod staff yn cael hyfforddiant priodol a chyfredol, a bod eu cymhwysedd yn cael ei gadarnhau'n ffurfiol gan weithiwr iechyd proffesiynol cymwysedig, lle bo hynny'n berthnasol.
Rheoli meddyginiaethau a chofnodion: Rhaid i ysgolion sicrhau bod meddyginiaethau'n cael eu storio, eu gweinyddu a'u dogfennu'n ddiogel, gan gynnwys prosesau diogel ar gyfer cael caniatâd priodol a chofnodi'r caniatâd hwnnw .
Cynlluniau gofal iechyd: Rhaid i ysgolion gynnal Cynlluniau Gofal Iechyd Unigol cyfredol ar gyfer pob dysgwr sydd angen cymorth gofal iechyd yn ystod y diwrnod ysgol. Rhaid i'r cynlluniau gynnwys rhif cyswllt clinigol brys a gweithdrefnau ar gyfer ymateb i argyfyngau, a rhaid iddynt gael eu cynnal i sicrhau eu bod yn parhau i fod yn gywir ac yn gyfredol.
Rolau a chymwyseddau staff: Rhaid i unrhyw dasgau gofal iechyd a wneir gan staff yr ysgol ddod o fewn gallu eu cymhwysedd, parodrwydd, hyder, hyfforddiant ac o fewn sicrwydd eu hyswiriant. Dylai staff yr ysgol ddeall bod hwn yn weithgaredd gwirfoddol, a'u bod yn gwbl ymwybodol o'r trefniadau yswiriant cysylltiedig a'r hyn y mae'r rhain yn ei olygu yn ymarferol. Ni ddylid rhoi unrhyw aelod o'r staff addysg mewn sefyllfa lle mae'n ofynnol iddynt wneud penderfyniadau clinigol neu gynnig barn glinigol.
Cynhwysiant dysgwyr: Rhaid i'r trefniadau gefnogi dysgwyr i gymryd rhan lawn ym mywyd yr ysgol, gan gynnwys darparu addasiadau rhesymol lle bo angen.
Cydymffurfio â gofynion yswiriant: Rhaid i drefniadau yswiriant sicrhau nad yw staff addysg (parhaol, dros dro neu wedi'u cyflogi drwy asiantaeth) yn ymgymryd â thasgau gofal iechyd sy'n dod y tu allan i gwmpas polisi'r awdurdod lleol. Fel arfer, nid yw polisïau yswiriant safonol yn darparu yswiriant ar gyfer mewnosod neu ailosod tiwbiau (cathetrau, botwm MIC-KEY, PEG, tiwb nasogastrig (NG)) neu ymyriadau penodol eraill, gan gynnwys peiriannau sugno, gofal traceostomi, peiriannau anadlu, aciwbigo, chwistrellu'r glust, a gosod pwmp chwistrell. Dylai cyrff llywodraethu sicrhau bod trefniadau yswiriant priodol ar waith. Gall yr awdurdod lleol ddarparu cyngor a, lle bo'n berthnasol, cadarnhau unrhyw sicrwydd yswiriant sydd gan lywodraeth leol.
Trefniadau ar gyfer ymateb i argyfyngau: Rhaid i ysgolion gynnal systemau ar gyfer rheoli argyfyngau gofal iechyd, ar y safle ac yn ystod gweithgareddau oddi ar y safle. Mae hyn yn cynnwys cael meddyginiaeth frys ac eglurder ynghylch rolau, cyfrifoldebau a gweithdrefnau uwchgyfeirio.
Os oes gan leoliad addysg neu awdurdod lleol bryderon, rhaid i'r pryderon gael eu huwchgyfeirio i'r bwrdd iechyd lleol. Bydd un pwynt cyswllt yn cael ei nodi a'i rannu.
Yr hyn a ddisgwylir gan y byrddau iechyd lleol
Mae'r disgwyliadau hyn yn berthnasol mewn ysgolion arbennig ac i blant sydd angen cymorth gofal iechyd mewn ysgolion prif ffrwd. Gellir crynhoi'r disgwyliadau hyn fel a ganlyn:
Darparu gwasanaeth nyrsio cyson sy'n bodloni safonau cenedlaethol ar draws pob lleoliad ar gyfer dysgwyr ag anghenion gofal iechyd sylweddol neu gymhleth.
Sicrhau gweithlu digon medrus a chefnogi datblygiad parhaus y gweithlu, sy'n unol ag anghenion dysgwyr.
Galluogi cydlyniant amlasiantaethol effeithiol, gan sicrhau cyfranogiad llawn yn y Tîm o amgylch y Plentyn/Teulu sy'n cyfrannu at gyfathrebu rheolaidd, cynllunio ar y cyd, a chymorth cydgysylltiedig ar draws addysg, iechyd a gofal cymdeithasol.
Cynnal llywodraethu cryf a sicrwydd ansawdd i sicrhau ymarfer nyrsio diogel o ansawdd uchel ar draws pob lleoliad addysgol.
Hyrwyddo tegwch a chysondeb darpariaeth ar gyfer pob dysgwr ag anghenion gofal iechyd sylweddol neu gymhleth. Mae hyn yn cynnwys sicrhau bod gan bob ysgol arbennig nyrs benodedig â'r sgiliau priodol.
Cefnogi'r gwaith o barhau i wella gwasanaethau i sicrhau bod dysgwyr ag anghenion gofal iechyd sylweddol neu gymhleth yn cael eu cefnogi'n briodol.
Darparu cyngor, arweiniad, hyfforddiant a chymorth ymarferol ac amserol i ysgolion i alluogi rheoli anghenion gofal iechyd yn ddiogel. Mae hyn yn cynnwys sicrhau bod cynlluniau gofal yn gyfredol ac yn darparu cyfarwyddyd clir i staff yr ysgol ar ofal y plentyn, ac ar gefnogi gweithredu cynlluniau gofal, datblygu'r galluogrwydd ynghylch cymorth sy'n gysylltiedig ag iechyd, a darparu un pwynt cyswllt i hyrwyddo cysondeb cael hyd i gymorth.
Cyfrannu gwybodaeth iechyd arbenigol i Gynlluniau Gofal Iechyd Unigol drwy ddarparu gwybodaeth glinigol, cydweithio ag ysgolion ac awdurdodau lleol, cefnogi gwaith drafftio lle bo angen, a darparu gwybodaeth wedi'i diweddaru wrth i anghenion newid.
Cefnogi'r gwaith o reoli meddyginiaethau yn ddiogel trwy ddarparu arweiniad clinigol a hyfforddiant sy'n cefnogi datblygu gweithdrefnau cadarn ar gyfer ymateb i argyfyngau.
Cefnogi rhannu gwybodaeth gyfreithlon ac effeithiol i sicrhau bod staff yn deall anghenion dysgwyr ac yn gallu ymateb yn ddiogel ac yn briodol.
Cefnogi cynllunio a chymorth ar gyfer pontio i sicrhau bod y cymorth gofal iechyd i ddysgwyr ag anghenion gofal iechyd sylweddol neu gymhleth yn parhau.
Dylai diogelwch unrhyw aelod o staff yr ysgol sy'n ymgymryd â thasg gofal iechyd gael ei benderfynu fesul achos gan weithiwr gofal iechyd proffesiynol priodol, a dylid cofnodi'r penderfyniad yn glir yng nghynllun gofal iechyd y plentyn. Dylai staff yr ysgol ymgymryd â thasgau gofal iechyd dim ond os bodlonir pob un o'r pedwar amod canlynol:
Mae gweithiwr gofal iechyd proffesiynol wedi gweithio gyda'r ysgol i ddatblygu cynllun gofal iechyd sy'n cynnig cyfeiriad clir ar y tasgau dan sylw a manylion cyswllt clinigol, ac yn cynnwys asesiadau risg sy'n benodol i anghenion y plentyn.
Ymgymerir â'r holl dasgau gan staff yr ysgol ar sail wirfoddol.
Mae'r bwrdd iechyd lleol wedi darparu hyfforddiant priodol i staff perthnasol yr ysgol, wedi cadarnhau eu cymhwysedd i gyflawni'r tasgau gofal iechyd, ac wedi sicrhau bod y cymhwysedd hwnnw'n cael ei gynnal trwy hyfforddiant a goruchwyliaeth barhaus sy'n seiliedig ar dasgau benodol.
Mae'r awdurdod lleol wedi darparu cadarnhad ysgrifenedig bod yswiriant digonol ar waith ar gyfer tasgau gofal iechyd perthnasol.
Os nad yw un neu fwy o'r amodau hyn yn cael eu bodloni, rhaid uwchgyfeirio pryderon ar unwaith i Gyfarwyddwr Gweithredol Nyrsio y Bwrdd Iechyd Lleol perthnasol er mwyn gwneud darpariaeth briodol, neu i Gyfarwyddwr Addysg yr Awdurdod Lleol, fel y bo'n briodol. Os bydd y pryderon yn cael eu huwchgyfeirio, bydd LlC yn disgwyl i'r bwrdd iechyd lleol a'r awdurdod lleol weithio gyda'i gilydd i gadarnhau trefniant amgen mewn pryd i osgoi tarfu ar bresenoldeb plentyn yn yr ysgol.
Mae gwaith ar y gweill i ddatblygu canllawiau integredig newydd ar gyfer partneriaid addysg ac iechyd i gefnogi dull cyson, diogel a theg o ddiwallu anghenion gofal iechyd dysgwyr ym mhob lleoliad. Bwriedir LlC i'r canllawiau newydd ddarparu fframwaith unedig sy'n cryfhau cydweithredu, yn egluro rolau a chyfrifoldebau, ac yn cefnogi darpariaeth o ansawdd uchel. Byddant hefyd yn nodi prosesau clir ar gyfer asesu ac ymateb i anghenion gofal iechyd plant a phobl ifanc mewn ysgolion, yn amlinellu llwybrau priodol ar gyfer uwchgyfeirio, ac yn disgrifio sut y bydd partneriaid yn gweithio gyda'i gilydd i gryfhau llywodraethu, ymdrin â phryderon yn well, a sicrhau bod gan y gweithlu fynediad at yr hyfforddiant a'r cymorth angenrheidiol. Bydd y canllawiau hyn yn cael eu datblygu gyda chyfraniad yr holl randdeiliaid perthnasol, gan gynnwys plant a'u teuluoedd. Rhagwelir LlC y bydd y canllawiau newydd hyn ar gael erbyn gwanwyn 2027.
Recently, Welsh Government has reiterated what it expects from schools, local authorities and local health boards in relation to the support provided to children and young people with healthcare needs in each educational setting in Wales, including the need to determine a clear policy standpoint on this matter. This is key to ensuring that children and young people with healthcare needs can receive safe and fair education without unnecessary disruption.
The intention of the policy expectations identified in this letter is to reiterate our commitment to the following:
Ensuring that learners with healthcare needs are not under disadvantage in terms of accessing education or taking part in school activities.
Establishing consistent arrangements of high quality to identify, plan and provide healthcare support in maintained educational settings.
Strengthening the collaboration between schools, local authorities, health boards and families.
Promoting safe and appropriate evidence-based practices when managing healthcare needs in maintained educational settings.
Ensuring that the rights, dignity and welfare of learners are always prioritised.
The guidance documents Supporting learners with healthcare needs (2017) and Part 2 of the School Nursing Framework (2018) identify the statutory and non-statutory responsibilities on schools, local authorities and local health boards in relation to how learners with healthcare needs should be supported. Together, these guidance documents describe how partners should work together to ensure that the needs of learners are clearly understood and that appropriate and interconnected support is put in place. A summary of the main responsibilities can be found below.
Welsh Government expects all partners to understand and fulfil these responsibilities, ensuring that the child's needs and best interests always remain central to the process.
What is expected from schools and local authorities
These expectations are relevant in special schools and for children who require healthcare support in mainstream schools. These expectations can be summarised as follows:
Healthcare policy: Every school must develop, maintain, and publish a Healthcare Policy outlining arrangements for supporting learners with healthcare needs. See the attached policy template.
Governance and training: Governance arrangements must be in place to ensure that staff receive appropriate and up-to-date training, and that their competence is formally confirmed by a qualified healthcare professional where applicable.
Medication management and records: Schools must ensure that medications are stored, administered, and documented safely, including secure processes for obtaining appropriate consent and recording that consent.
Healthcare plans: Schools must maintain up-to-date Individual Healthcare Plans for every learner requiring healthcare support during the school day. Plans must include an emergency clinical contact number and procedures for responding to emergencies, and must be kept under review to ensure they remain accurate and current.
Staff roles and competencies: Any healthcare tasks carried out by school staff must be within the limits of their competence, willingness, confidence, training, and insurance cover. School staff should understand that this is a voluntary activity, and they must be fully aware of the associated insurance arrangements and what these mean in practice. No member of education staff should be placed in a position where they are required to make clinical decisions or provide clinical judgement.
Learner inclusion: Arrangements must support learners to participate fully in school life, including providing reasonable adjustments where necessary.
Compliance with insurance requirements: Insurance arrangements must ensure that education staff (permanent, temporary, or agency) do not undertake healthcare tasks that fall outside the scope of the local authority’s policy. Standard insurance policies typically do not cover the insertion or replacement of tubes (catheters, MIC-KEY button, PEG, nasogastric (NG) tube) or certain other interventions, including suction machines, tracheostomy care, ventilators, acupuncture, ear syringing, and syringe pump use. Governing bodies should ensure that appropriate insurance arrangements are in place. The local authority may provide advice and, where appropriate, confirm any insurance cover held by local government.
Emergency response arrangements: Schools must maintain systems for managing healthcare emergencies both on-site and during off-site activities. This includes having emergency medication available and clear definitions of roles, responsibilities, and escalation procedures.
If an educational setting or local authority has concerns, the concerns must be escalated to the local health board. One point of contact will be identified and shared.
What is expected from the local health boards
These expectations apply in special schools and to children who require healthcare support in mainstream schools. These expectations can be summarised as follows:
Provide a consistent nursing service that meets national standards across all settings for learners with significant or complex healthcare needs.
Ensure a sufficiently skilled workforce and support the ongoing development of that workforce, in line with the needs of learners.
Enable effective multi-agency collaboration, ensuring full participation in the Team Around the Child/Family, contributing to regular communication, joint planning, and coordinated support across education, health, and social care.
Maintain strong governance and quality assurance to ensure safe, high-quality nursing practice across all educational settings.
Promote equity and consistency of provision for all learners with significant or complex healthcare needs. This includes ensuring that every special school has a designated nurse with the appropriate skills.
Support ongoing service improvement to ensure that learners with significant or complex healthcare needs are appropriately supported.
Provide timely advice, guidance, training, and practical support to schools to enable the safe management of healthcare needs. This includes ensuring that care plans are up to date and providing clear guidance to school staff on the child’s care, supporting the implementation of care plans, developing capability in relation to health-related support, and providing a single point of contact to promote consistent access to support.
Contribute specialist health knowledge to Individual Healthcare Plans by providing clinical information, working collaboratively with schools and local authorities, supporting drafting where necessary, and providing updated information as needs change.
Support safe medication management by providing clinical guidance and training that supports the development of robust emergency response procedures.
Support lawful and effective information sharing to ensure that staff understand learners’ needs and can respond safely and appropriately.
Support transition planning and provision to ensure continuity of healthcare support for learners with significant or complex healthcare needs.
The safety of any member of school staff undertaking a healthcare task should be determined on a case-by-case basis by an appropriate healthcare professional, and the decision should be clearly recorded in the child’s healthcare plan. School staff should undertake healthcare tasks only if all four of the following conditions are met:
A healthcare professional has worked with the school to develop a healthcare plan that provides clear guidance on the tasks involved and clinical contact details, and includes risk assessments specific to the child’s needs.
All tasks are undertaken by school staff on a voluntary basis.
The local health board has provided appropriate training to relevant school staff, confirmed their competence to carry out the healthcare tasks, and ensured that this competence is maintained through ongoing, task-specific training and supervision.
The local authority has provided written confirmation that adequate insurance cover is in place for the relevant healthcare tasks.
If one or more of these conditions are not met, concerns must be escalated immediately to the Executive Director of Nursing of the relevant local health board in order to make appropriate provision, or to the Director of Education of the Local Authority, as appropriate. Where concerns are escalated, the Welsh Government will expect the local health board and local authority to work together to confirm an alternative arrangement in a timely manner to avoid disruption to a child’s school attendance.
Work is underway to develop new integrated guidance for education and health partners to support a consistent, safe, and equitable approach to meeting learners’ healthcare needs in all settings. The Welsh Government intends for the new guidance to provide a unified framework that strengthens collaboration, clarifies roles and responsibilities, and supports high-quality provision. It will also outline clear processes for assessing and responding to the healthcare needs of children and young people in schools, describe appropriate escalation pathways, and explain how partners will work together to strengthen governance, better address concerns, and ensure that the workforce has access to the necessary training and support. This guidance will be developed with input from all relevant stakeholders, including children and their families. The Welsh Government anticipates that the new guidance will be available by spring 2027.
