Medications

Medications can be administered at school provided the current procedures are followed. Links are posted to the left to include the district's policy and the Medication Order/Permission Form.

Please do not send any medication to school with your child. A parent or guardian should bring medication to the nurse's office. This allows for seamless communication about the administration of the medication in school and keeps other children safe as well.

Written permission from both a parent and your child's physician is required for any prescription medication to be given during school hours or during school sponsored events. For over the counter medications, written parental permission is required.

District Policy

Medication Order & Permission Form

A school nurse shall assure that in the administration of medication, the following procedures are followed:

1. A written order must be obtained from a medical provider detailing the name of the medication, dosage, time the medication is to be given, the date of termination for administration, and the diagnosis and/or reason for giving the medication at school (For ALL Prescription Medication).
2. A written permission from the parent/guardian of the student must be obtained (For ALL Prescription and Non-Prescription Medications).
3. Medication must be brought to school in a container appropriately labeled by the pharmacy or health care provider.
4. Delivery of all medications to elementary schools is the responsibility of the parent/guardian or other designated adult. Medication should not be transported to and from elementary school grounds by students.
5. The initial dose of a medication will not be given at school.
6. Medications shall be stored in a locked compartment in the health office.
7. Administration of medication shall be recorded on a Medication Log.
8. The parent/guardian of elementary students needs to pick up unused medication within three (3) days following the termination date of administration. The school nurse retains the right to discard unused medication following this three (3) day period.
