Secondary School Process

Concussion Management Team (CMT)

Point Person (Lead) - appointed by building principal

• Makes sure process is followed in a timely manner

• May assign someone as plan manager or do it themselves

• Trains new teammates and answers questions 

Plan Managers (Oversees the case) - recruited and approved by building principal;
    has some flexibility in their schedule and 10 minutes a day that can be used for team tasks

• Interviews the family and/or student on current symptoms and what accommodations may help

• Connects weekly with athletic trainer for physical activity restrictions, if applicable

• Gathers any feedback from teachers

• Gives draft ITAP to counselor

• Adds counselor approved ITAP to a CMT folder and updates CMT spreadsheet

Counselor (Accommodation expert)

• Approves suggested accommodations

• Emails out the finalized ITAP

Teacher, Student, Family 

• Provides feedback to plan manager on observations and suggestions on accommodations
