Concussion Management Team (CMT)
Point Person (Lead) - appointed by building principal
Makes sure process is followed in a timely manner
May assign someone as plan manager or do it themselves
Trains new teammates and answers questions
Plan Managers (Oversees the case) - recruited and approved by building principal;
has some flexibility in their schedule and 10 minutes a day that can be used for team tasks
Interviews the family and/or student on current symptoms and what accommodations may help
Connects weekly with athletic trainer for physical activity restrictions, if applicable
Gathers any feedback from teachers
Gives draft ITAP to counselor
Adds counselor approved ITAP to a CMT folder and updates CMT spreadsheet
Counselor (Accommodation expert)
Approves suggested accommodations
Emails out the finalized ITAP
Teacher, Student, Family
Provides feedback to plan manager on observations and suggestions on accommodations
