The brain is a very delicate organ. It is roughly the consistency of gelatin. Natural protection includes a wrap of tissues (meninges), cushion from floating in cerebrospinal fluid, and encasement in a bony skull.
Injuries come from direct impact, rocking of the brain in the skull case, and concussive blast injuries.
Injury to neurons, even when widespread, is not visible on MRI or CT scan.
TBI diagnosis and severity is estimated by symptoms, functional assessments, and associated complications (like skull fractures, brain hemorrhages, etc.)
