The following two items are also needed if your student is new to the Hillsboro School District or is coming from homeschool, a private school, or a charter school. Original documents should be brought to our Oak Street Campus (address above) and copies will be made for your student's file.

Up-to-Date Immunization Record
Proof of Birthdate (i.e. birth certificate, passport)
