Club Announcement Form

• The Club Announcement Form must be submitted to Ms. Matsu from the club sponsor. No exceptions.

Record of Community Service

• Sponsors must be in agreement with the number of community service hours issued.

• The number of community service hours earned must be commensurate with the number of community service hours performed.

• Community service hours cannot be given for donations.

• Students must complete 100 hours of community service by April 1st of their senior year in order to meet the community service requirement.
