Qualified Mental Health Professional
H & N always welcomes experienced QMHP-C's to provide intensive in-home services for our youth and families. QMHP's provide direct in-home services that implement a service plan which provides structure, support, and services to individuals at risk for removal from the home. The QMHP will also provide counseling services as required to include individual and family counseling, assist our clients in meeting individualized treatment goals as outlined in their treatment plans, provide linkages to various community resources, and work directly with program participants and other collaborative services to ensure client’s needs are met. Ideal candidate will possess at least a bachelor's degree in the human services field or special education from an accredited college and with at least one year of clinical experience with children and adolescents.
Direct Support Professional
At H & N Services LLC, Direct Support Professionals (DSPs) play a vital role in enhancing the quality of life for children and youth in our residential group home setting. DSPs are responsible for providing compassionate care and personalized support to individuals with intellectual, developmental, or physical disabilities. This includes assisting with daily living activities such as personal hygiene, meal preparation, dressing, and mobility, while promoting independence and skill-building. DSPs at H & N Services also help clients develop life skills, manage behaviors, and engage in community activities to encourage social inclusion. They monitor client health and safety, assist with medication administration, and ensure that individualized support plans are followed effectively. Documentation of services, progress, and incidents is a critical part of the role, as is advocating for the needs, rights, and preferences of each client. Through their dedication, DSPs help foster a nurturing and empowering environment where clients can thrive.
Case Manager
The Case Manager at H & N Services LLC plays a pivotal role in ensuring the delivery of high-quality, person-centered care for children residing in our Developmental Disabilities (DD) residential group home. The Case Manager is responsible for coordinating services, developing individualized service plans (ISPs), and collaborating with families, school personnel, healthcare providers, and other stakeholders to meet the unique needs of each child. This role involves conducting assessments to identify strengths and needs, monitoring progress, and advocating for resources and supports to enhance the child’s development and well-being. The Case Manager oversees compliance with state and federal regulations, facilitates meetings with families and service providers, and maintains detailed documentation of all services and communications. By fostering a collaborative and supportive environment, the Case Manager ensures that each child has the tools and opportunities to achieve their full potential.
