It is District Policy that ALL student medication (prescription & non) including inhalers, require a doctor’s order. Most medications are stored in the nursing office. With parent permission/signature and doctor’s instruction, students may be given medication by a nurse and/or school employee. All medication forms should be turned in by the first day of school (Medication in school order is renewable every school year).
You will need to complete and sign this form and have your student’s physician authorize the medication. Remember all medications (even those over-the-counter) require a completed form.
*Pain Relievers: We do not keep stock Tylenol, Ibuprofen, or any other pain relieving medication on campus. Students are NOT allowed to carry this medication on them. It will be held in the nurse office and when needed the student may request it from the nurse, so that it can be supervised and given safely.
*Emergency medication such as Epi-Pen are kept on each campus to administer to students, staff, and visitors, if needed. We do NOT stock emergency medication for asthma.
Should your student need medication for a field trip or school sponsored extracurricular activity, please contact your site nurse for arrangement (see Home page).

Medication WILL NOT be given until this form is completed and on file in the school health office.
A parent/guardian must bring the medication to the school and pick up any outdated, unused or for home use medication.
All medication must be in a container labeled by a pharmacist or prescribing physician.
A current medication form must be on file. A new form for each medication must be completed and on file for each school year.
Letter C on this form MUST be completed in order for the student to self carry and administer emergency medication at school. Otherwise the the Licensed Health Care Provider or trained volunteer will have to carry and administer.
Parents/Guardians must provide all materials or necessary equipment for medication Administration.
A copy of this Medication Order must be provided by the physician to the school nurse, school administrator and/or unlicensed volunteer.
Changes in prescribed dose and other details of medication administration must be provided to the school nurse, school administrator and/or unlicensed volunteer, in writing, by the delegating physician.
All medication not picked up by a parent/guardian on the last day of school will be discarded in accordance with district policy.
