HEALTH CARE PLANS

Students with health conditions may have a health care plan created at the beginning of each school year and emailed to teachers. It is important that during the registration process, all health concerns are documented on the Student Health Update form or you contact the School Nurse with any new health conditions.


If your child needs ANY accommodations, exemptions, or has restrictions at school a doctor's note is needed with an END DATE. (Ex. For the School Year 2023-24; 2 months, 2 weeks etc.). A doctor's note for an accommodation is renewable every school year.

Some of the health conditions that may require a health care plan include (but not limited to):

aSTHMA

A condition in which a person's airways become inflamed, narrow and swell, and produce extra mucus, which makes it difficult to breathe.

If your child has a self carry and administer order we still need the order for school. Please remind your child to carry the emergency inhaler with them at all times, especially during PE. The health office staff can also carry an extra inhaler in their emergency bag.

If your child has asthma and needs an inhaler at school please complete the Asthma Action Plan (below) and Medication Administration Request.


aLLERGIES

An allergy is a heightened sensitivity to a foreign substance that causes the body's defense system to overreact when defending itself. The severity of an allergic reaction can vary from mild discomfort to life threatening situations.

Please note that our cafeteria does serve peanut products and other items that may cause your child to have an allergic reaction. Please educate your child on how to read ingredient lists on packaging and be aware of the foods they are eating. Also, educate your child on the signs of an allergic reaction and when they should see the nurse.

If you child has allergies please complete the Allergy Action Plan (below).  If your child is allergic to food please complete the Meal Accommodation Form.  In addition, if specific medications are necessary, such as antihistamine or epinephrine please have your physician complete the Medication Administration Request.

EPILEPSY/SEIZURES

Based on the type of behavior and brain activity, seizures are divided into two broad categories: generalized and partial (also called local or focal). If you child has a history of seizure activity please complete the Seizure Action Plan (below).

Please remember to keep our health staff updated on any recent seizure activity or changes in medications.

DIABETES

Students with either insulin dependent Diabetes, or Diabetes that is controlled with oral medication will meet with the school nurse and school site team annually to review and updated healthcare plans.  Each school year, we require that your physician provide the school district with updated medical orders. 

Please remember to provide juice and snacks for you child to carry with them and have extra in the health office in the event of hypoglycemia.
