It is important for students to feel prepared to transition at these pivotal moments of their schooling, not only for themselves but for the new team supporting them.  Consider creating a a tool for your students to introduce themselves to their new teachers/service providers/case managers in a way they are comfortable.  Develop this with your student to give them voice about what they would like their new team to know and allow them to communicate what has been the most supportive for them while in your charge.
