Everyone needs 5 forms to try out for any sport 2026 - 27
The high school VHSL physical form (3 pages turned in) is used in Middle School too. Physicals must be signed by a doctor and parent and must include insurance. If you need insurance, please contact Ms. Schooley
Physicals last for 14 months from the date they were given. Please be aware that your physical may expire in the middle of a season now and may need to be updated to finish the season.
