Everyone needs 5 forms to try out for any sport 25-26
The high school VHSL physical form (3 pages turned in) will now be used in Middle School too. Physicals must be signed by a doctor and parent and must include insurance.
NEW this year: Physicals last for 14 months from the date they were given. Please be aware that your physical may expire in the middle of a season now and will need to be updated to finish the season.
