Sports Physical

Students who wish to participate in any intramural activity, including dance or judo, must have a current physical examination completed within 364 days prior to the start of the school year.

The physical examination must be done by your own physician or health care provider. The form is required by the N.J. Department of Education and must be utilized by your physician. The student will not be allowed to try out or participate in the sport unless the form is complete.

Important: Please Follow These Instructions Carefully

2. Give the following pages to your child’s doctor to complete during the physical exam:

   • History Form – Pages 2 and 3

   • Athletes with Disabilities: Supplement to the Athlete History – Page 4

   • Physical Examination Form – Page 5

   • Preparticipation Physical Evaluation Medical Eligibility Form – Page 6
     
     

3. After the exam:

   • You must return the completed and signed “Preparticipation Physical Evaluation Medical Eligibility Form” (Page 6) to your child’s school nurse.
     
     

Only Page 6 needs to be submitted to the school. The other forms are for your records and your physician’s reference.
