Print all required information in blue or black ink.
Send a copy of the form to Tina Fix, HR Information Systems Specialist (via pony to Central Office or email a scan of the completed form to cfix@hcps.us) and to the administrative assistant of your department or school.
If requesting a legal name change, please send a copy of your updated Social Security Card or the letter from the Social Security Office along with the form. The name change can not be processed without this.
