To change your legal name, marital status, phone number(s), address, or emergency contact information follow the steps below

1. Click this link and print the form.

2. Print all required information in blue or black ink.

3. Send a copy of the form to Tina Fix, HR Information Systems Specialist (via pony to Central Office or email a scan of the completed form to cfix@hcps.us) and to the administrative assistant of your department or school.

4. If requesting a legal name change, please send a copy of your updated Social Security Card or the letter from the Social Security Office along with the form. The name change can not be processed without this.
