All new field instructors are required to complete this form. Additionally, this form must be submitted by field instructors who have changed agencies or are resuming their role after a hiatus from supervising students.

New field instructors may also get a request form Exxat to update your profile in the system. Note that not all fields are mandatory, please add what would be relevant to your role as a field instructor.

This orientation provides field instructors and site supervisors with updated field education policies. Major changes include a revised BSW spring hour requirement, elimination of the mid-semester field evaluation, mandatory comment boxes in the evaluation and grading form, and automated grading form delivery via Exxat.
