SOR-4 Data Collection Training

Instructions

• Access the e-learning course by entering your work email address in the Sign-In form.

• Complete the SOR-4 data collection e-learning course below. This course is approved for 1.5 CE hours by ADAD. 

• Complete the 20-item quiz and obtain a score of 16 or higher (≥80%). Take the quiz again if needed.

• Submit your score, name, email, and organization through the Quiz Result form.

• Check your email for the Certificate of Completion upon successful completion of the training (with a score of 16 or higher).
  Note: Look for the email from noreply@hawaii.edu in your inbox or spam/junk folder.
