This is a demo site for training purposes.
Review the table below to locate your client’s Episode of Care (EOC) record.
If your client doesn’t have an EOC record, create one via the "Add New EOC Record" button. Refresh your browser to see the new record added.
On your client's EOC, click the appropriate link to open the assessment tool for intake/baseline, 6-month follow-up, or discharge assessment.
There should be only one active, open EOC for a client at any time. Contact the Data Team if you need any assistance.
Once a GPRA is entered, the submission will be reflected on the EOC record within 15 minutes.

color-coded:
