After you receive your approval to apply, please send a copy to the Secretary to the Chancellor Allison Ohata at amo3111@hawaii.edu. Allison Ohata is the campus coordinator and will need the approved eligibility form in order to create your online dossier account.

For Contract Renewals, please be reminded that the Service Data Contract Renewal form is REQUIRED. Please select the correct Contract Renewal form that applies to you; probationary for tenure-track, non-probationary for non-tenure track, acting if you are an Acting Instructor. Complete the service data including this 2024-2025 academic year along with the requisite signatures (yours and your supervisor's) for upload.
