Health Information - School Entry Health Requirements

SCHOOL ENTRY HEALTH REQUIREMENTS

All children at least 4-5 years old must have:

5 doses of DTaP or DTP (diphtheria, tetanus, pertussis)
4 doses of IPV or OPV (polio) or combinations of the two vaccines.
3 doses of hepatitis B.
2 doses of MMR (measles, mumps, rubella).
2 doses of varicella (chickenpox) or verification of a history of varicella disease by a health-care provider
2 doses of hepatitis A

- Sixth Grade Entry Immunization Requirements:

In addition to the 4-5 year old immunizations, students age 11 or 12 years or older, shall have received:
