Nurse and Medication Consent Forms

School Health Nurse Consent Form 2025-26

• Health Consent Form must be completed for nurse to assess and treat / give medication(s) to your child. 

Prescription / Over the Counter Medication Consent Form

• Only one medication per form

• Must be completed for ALL medications given at school: daily ADHD medications; emergency medications, short term medications such as antibiotics or steroid packs, as well and ANY Over the Counter medications you wish the nurse to give to your student (this is a new state policy)

• Prescription Medication Consent form must be completed by Health Care Provider and Parent / Guardian 

• Medication will not be accepted without completed form for current school year

• All prescription medication brought to school must be in original container with current prescription label attached and NOT expiring (if possible) prior to school year ending.

• If your child will be carrying an inhaler, please put their name on it, in the event they misplace it at school.

• ***Students may only carry Emergency Medication (inhaler, epi-pen, seizure) that the doctor & parent has signed consent to carry. Students may not carry any other medications i.e. tylenol, ibuprofen, ADHD medications.***
