Nurse and Medication Consent Forms

School Health Nurse Consent Form is now in the Online Registration (OLR)

• Consent to see the nurse must be marked Yes for the school nurse to assess and treat / give medication(s) to your child. 

Prescription / Over the Counter Medication Consent Form

• Prescription / Over the Counter Medication Consent form must be completed by both a Health Care Provider and Parent / Guardian EACH school year.

• Must be completed for ALL medications given at school: daily scheduled medications such as ADHD medicine; emergency medications, short term medications such as antibiotics or steroid packs, as well and ANY Over the Counter medications brought to school for the nurse to give to your student   (per KDE policy effective July 1, 2025).

• Only one medication per form.

• Medication will not be accepted without completed form for current school year.

• All Prescription and Over the Counter medications brought to school must be in the original container with current prescription label (if a prescription) attached and medicine must NOT be expired. If medication expires during the school year, a new Prescription or Over the Counter medication must be brought to school to replace the expired one.

• If your child will be carrying an Emergency Medication such as an inhaler for self administration, please have the pharmacy put a prescription label directly on the inhaler. Also have the provider complete the "For Self Administration Only" section on the consent.

• ***Students may only carry Emergency Medications (inhaler, epi-pen, seizure) that the doctor & parent has signed consent to carry. Students may not carry any other medications i.e. tylenol, ibuprofen, ADHD medications.***
