School Health Nurse Consent Form 2025-26
Health Consent Form must be completed for nurse to assess and treat / give medication(s) to your child.
Prescription / Over the Counter Medication Consent Form
Only one medication per form
Must be completed for ALL medications given at school: daily ADHD medications; emergency medications, short term medications such as antibiotics or steroid packs, as well and ANY Over the Counter medications you wish the nurse to give to your student (this is a new state policy)
Prescription Medication Consent form must be completed by Health Care Provider and Parent / Guardian
Medication will not be accepted without completed form for current school year
All prescription medication brought to school must be in original container with current prescription label attached and NOT expiring (if possible) prior to school year ending.
If your child will be carrying an inhaler, please put their name on it, in the event they misplace it at school.
***Students may only carry Emergency Medication (inhaler, epi-pen, seizure) that the doctor & parent has signed consent to carry. Students may not carry any other medications i.e. tylenol, ibuprofen, ADHD medications.***
