Health Consent Form must be completed for nurse to assess and treat / give medication(s) to your child.Â

Only one medication per form
Prescription Medication Consent form must be completed by Health Care Provider and Parent / Guardian
All prescription medication brought to school must be in original container with current prescription label attached and NOT expiring (if possible) prior to school year ending.
If your child will be carrying an inhaler, please put their name on it, in the event they misplace it at school.
***Students may only carry Emergency Medication (inhaler, epi-pen, seizure) that the doctor & parent has signed consent to carry. Students may not carry any other medications i.e. tylenol, ibuprofen, ADHD medications.***
Over-The-Counter Medication Consent Form
(Or Short-Term Medication: Antibiotics, etc)
Only one medication per form
All over the counter medication brought to school must be in unopened original container and NOT expiring prior to the end of the school year and will be kept with the school nurse.
All prescription medication brought to school must be in original container with current prescription label attached and NOT expiring (if possible) prior to school year ending.
