Address Change - The fastest way to change your address is online via Workday. If you cannot access Workday, please use this form.
Name Change - Legal name and gender changes can be made, with appropriate supporting documentation, in Workday. If you cannot access Workday, please use this form.
Flexible Curriculum Proposal - A student may design their own major with approval of this form. Must be submitted no sooner than the first term of the sophomore year and no later than the first term of the senior year.
Grade Appeal Request Form - Students may appeal the grade assigned to them in a course. Submission of an appeal does not guarantee a grade change. See the complete grade appeal policy in the Bulletin (bulletin.hamline.edu).
Leave of Absence Request - All Leave of Absence requests are now in Workday. Go to the Academics Application and look under General Help.
Request for Incomplete Grade - Students unable to finish a course due to special circumstances may request an incomplete grade from their instructor.
Undergraduate Petition of Academic Policy - Request an exception to academic policy by submitting a petition to the Undergraduate Curriculum Committee. Submission of a petition does not guarantee approval.

Bar Authorization Form - Include this form with your bar certification request. It allows us to provide the necessary documentation to Boards of Law Examiners/State Bars/Equivalent Licensing Agencies.
Undergraduate non-Hamline Visitor Registration Form - Visiting students use this form to register for undergraduate courses.
ACTC Cross-Registration - Undergraduate form to request registration at one of Hamline's ACTC partner schools.

Additional License Advanced Practicum Registration (OLD) - For students who were admitted to the additional license program prior to Fall 2015.
Additional License Advanced Practicum Registration (NEW) - For students who were admitted to the additional license program in Fall 2015 or later.
Apprentice Teaching Contract - Undergraduate students may earn a maximum of 4 credits toward graduation by assisting faculty with their teaching responsibilities. Apprentice teaching is graded on the Pass/No Pass scale.
Audit Registration (Undergraduate) - Register for a course on a no-credit, ungraded basis for a flat fee. This form is also for visiting Amity Scholars.

Collaborative Research Registration - Collaborative research projects offer undergraduate students the chance to pursue independent study in greater depth with a faculty member, often in conjunction with the professor’s own research.
Cross-School/Cross-Program Registration - Complete this form to register for a course outside of your school or program.
Departmental Honors Project Proposal - Students with a minimum 3.25 GPA may submit proposals by May 1st of the spring term of their junior year.
Independent LEAP Agreement - Undergraduate students use this form to apply to add the LEAP designation to one of the following types of experiences: Apprentice Teaching, Collaborative Research, or Independent Study. NOTE: This form is NOT to be used with internships!

Individual Study Registration Form (Undergraduate) - If a class is not offered during the term the student needs to take it, students may take the class on an individual study basis with instructor permission. Instructors work individually with students to complete the coursework.
