NEW IMMUNIZATION REQUIREMENTS
All students attending in building, virtual or transferring into the 11th grade are required by law to show proof of: one booster dose of the meningococcal conjugate (MCV4) vaccination. If the first dose was received on or after their 16th birthday, a booster is not required.
Please provide Nurse Smallwood with a new Georgia Immunization 3231 form which can be obtained by contacting your child’s Primary Care Provider or Health Department. Thank you for your attention to this matter. For more information, please contact Nurse Smallwood.
