Texas Required Yearly Student Assessments

The state of Texas requires that all public school nurses evaluate all Pk, K, 1st grade students, 3rd grade students, fifth grade students, 7th grade students, 9th grade students, all new enrolled students and students that require assessment for hearing and vision.All 1st grade students, 3rd grade students, 5th grade students, 7th grade students and 9th grade students will be evaluated, by the school nurse, for acanthosis nigricans (ANTES). If a student is found positive, a letter will be generated to parents to encourage them to have their child evaluated by a Dr.All 5th gr. female students, 7th gr. female students & all male 8th gr. students will be evaluated for scoliosis, by the school nurse.

GPISD Clinic Procedures Policy

A student must have a Health Referral Form from his/her teacher to visit the nurse’s office. Students can not call parents to pick them up without being evaluated by the school nurse 1st. If the nurse determines that the child should go home, the nurse will contact the parent. A nurse will be available during school hours for illness or injury and to assist in health assessments and planning. Please notify the school nurse if your child has any health problems, special needs, allergies or is taking any medications. It is imperative that the school is given an emergency telephone number or contact person to reach in an emergency.


Emergency Medical Treatment

Parents shall complete the emergency card form each year that includes a place for parental consent for school officials to request medical treatment for the student, as provided by law. Parents shall also be asked to supply other information that could be required in case of an emergency; parents should update this information as often as necessary. The cost of services provided by ambulance, private physician, clinic, hospital, or dentist remains the responsibility of the parent or guardian and shall not be assumed by the Galena Park Independent School District. All UIL students do have additional school insurance that assists in any medical costs obtained through UIL functions.
