How to set up a Google Form to capture Conversation and Observation Data rather than relying solely on products (or an insurmountable volume of products for an Adapted model),
You can use an addon called Doc Appender to funnel evidence of learning from observations and conversations with each student into one individual doc per student.
In addition to providing you with assessment data, this could be shared with the student (and/or parent) as ongoing descriptive feedback that could be used to inform your assessment of the overall expectations.

Option 1 - Play with data input
Open the Spreadsheet
Go to the FORM (PLEASE DO NOT EDIT THE FORM - YOU ARE ABLE TO OPEN FOR EDITING IN ORDER TO MAKE A COPY)
Enter some random "data", and remember which student(s) you were "tracking"
Go to the spreadsheet and look at the "raw" data and the data on the "Student" page
You will note that the "class" page does not update until the teacher enters the most recent/consistent level in the top row using professional judgement (not a raw average).
Option 2 - Explore the Spreadsheet / Form and how it is programmed OR Add your class information!
Open the Spreadsheet
Make a copy into a folder on your drive.
A copy of the form should also be copied and automatically linked to the new spreadsheet.
