Athletic Registration will now be completed in the PowerSchool Parent Portal.
Navigate to Student Forms > General Forms > Athletics Registration Form
Complete all required fields
Upload a valid physical to complete the registration
A valid physical is a physical that has been completed within the past 13 months. We will accept a G Form OR CT Health Assessment Form.
Follow these Instructions for uploading documents in order to reduce the file size.
Please Note: When submitting your physical, we only require one (1) submission of either the G form or the CT Health Assessment Form. It is not necessary to submit these items if they are on file already. Please note that the nurses office and the athletic department are separate offices, and we do require that a valid physical is filed in the athletic department, in order for the student-athlete to participate.

Step 1: Sign up Online via PowerSchool and submit all required forms.

We are using the G form as an additional option, this is the form Glastonbury High School uses.
