Parents & Guardians: Please complete the Physical Forms/District Polices (Complete Pages 1-10) prior to physical examination.
For student-athletes planning to play the following Fall Sports:
\Grades 7-12 Boys Basketball
Grades 7-12 Girls Basketball
Grades 7-12 Coed-Wrestling
Grades 9-12 Bocceball
Grades 7-12 Musicial
AThe PIAA and Greater Johnstown School District require all student-athletes to have a physical prior to playing any sport. Physicals are good from May 1 of the current year to April 30. Furthermore the GJSD requires parents and guardians to complete necessary paper work prior to becoming an active participant in a sport/music program.

Commonly Asked Questions Physicals
Q: Can my student-athlete play without a physical?
A: No. A student-athlete must have a physical done and completed prior to the first day of mandatory workouts. If a student-athlete does not have a valid physical, they can not participate until the Johnstown Athletic Trainer has a copy of the physical documentation and forms.
Q: Does my student-athlete have to get a physical for every sport they play?
A: No. A student-athlete just needs one physical per athletic year (May 1 of current year to April 30 of current year).
Q: What is the cost of a physical?
A: All physicals are free to students who attend physicals on the announced date(s) at GJSD. If a parent or guardian chooses to have a physical conducted outside the GJSD scheduled date(s), the family is responsible for their own payment.
