The Arizona Long Term Care System (ALTCS) provides culturally competent care for disabled and elderly American Indians who are living on-reservation or who have lived on a reservation prior to admission into a nursing facility located outside of the reservation. ALTCS provides institutional care and home and community-based services to AHCCCS members who are at risk of institutionalization.

Attendant Care (also known as Direct Care Services)   Services that help with dressing, bathing and other personal care, housekeeping, meal preparation and supervision for those who are not safe to be left alone.  A member’s spouse can be the paid Direct Care Worker (with limits).  Talk to your case manager about conditions that apply to this service.

“Traditional” Attendant Care – a model of service delivery in which a provider agency of the member’s choice is authorized by the case manager to provide care.  The Direct Care Worker is an employee of the agency.  The Direct Care Worker may have been specifically chosen by the member to be the Direct Care Worker and/or may be a relative of the member, including the spouse

AHCCCS covers attendant care services provided to ALTCS members.  The intent of attendant care is to initiate strong support for keeping members integrated with their families, communities and other support systems.  This service requires involvement from the member/guardian/ designated representative in decisions related to attendant care provider functions. 

Gishey's Patient Care LLC is a certified trainer and tester for Direct Care Workers.  Gishey's Patient Care LLC provides the training needed to become an Attendant Care Worker (also called Direct Care Worker).

Thank you for taking an interest in Gishey’s Patient Care LLC, In-Home Care Services.   The first step, fill out the Request for Application for AHCCCS Long Term Care Services.  A Representative can fill out the application for the member applying for Arizona Long Term Care Services or ALTCS.  

1. Fill out Request for Application for AHCCCS Long Term Care Services

2. Return to Gishey’s Patient Care LLC

3. Visit from Navajo Nation ALTCS Case Manager and Nurse (assessment for number of hours of care needed.)

4. Case Managers will then assign your Home Care Agency.  Request for Gishey’s Patient Care LLC or Case Managers will assign you to another home care agency.  The member applying has a choice of agency. 

If you are planning on becoming a caregiver (Attendant Care Worker) for the new member, you will need to fill out the Attendant Care Worker Application.  Also, start working on getting Attendant Care Worker Qualifications completed before starting employment.
