Vaccines

We are immunized by vaccines.

We are all about ensuring our population have current vaccine records.

During this time we will:

1. look for records on the State of Arizona centralized record system

2. contact parents of students who are non-compliant regarding their vaccine record

3. send out a few email reminders

Gilbert Public Schools sets an exclusion date [typically during the last week of August] each year where if records have not been received the non-compliant student

will be excluded from classes until records have been received.

• Please ensure your student's name, date of birth, and provider's signature [when records are hand written] are present on their record.

• When furnishing records, they may be emailed or faxed for your convenience. This information is found on our "Home" page [link below].

Medication Intake

We will accept daily and as needed [PRN] medications at this time [and throughout the school year].

Hearing and Vision Screenings

Hearing and vision screenings are conducted on all students in grades:

PS, K, 3, 5. Grade 1 will receive hearing only.

In addition to new students, IEP students, and parental requests.
